school clipart

Augusta Independent and the Bracken County Health Department are working together with multiple community partners to establish a School Based Health Center. The proposed School Based Health Center is a health care facility to be located at the Bracken County Schools FRYSC office and open to the community and both school districts.

The intent is to fill a gap and add another option to obtain health care in the community and schools with a model similar to an urgent care. The proposed School Based Health Center will provide health assessments/treatment for minor ailments, immunizations, sports/CDL physicals, dental screenings/services, eye exams/services, mental health assessments/treatment, addiction counseling, etc. Furthermore, virtual office visits/telemedicine is a possibility.

Please take the short survey to give valuable information as a parent or community member to help guide the planning of the health care needs for everyone.

Thank you in advance for your participation and completion of the survey.
